To request an Emergency Medical Services (EMS) report or health record you must upload a completed Health Insurance Portability and Accountability Act (HIPAA) Privacy Authorization Form. Please note, receiving these records requires a notarized signature. Notaries are available at City Hall, 100 Civic Center Parkway, Monday - Friday 8 a.m. - 4:30 p.m.
